- Confirm the written request and date received.
- Calculate five calendar days from receipt.
- Select a specific physician—not merely a clinic or facility.
- Confirm the physician is not professionally affiliated with the prior physician.
- Send written authorization identifying the alternate physician by name.
- Move the appointment forward without unreasonable delay.
- Missed the 5 day deadline? Contact one of our attorneys to discuss options for handling.
